Life Skills (Certified Home Health Aide, Nutritional & Environmental Services, and Patient Support Technician/ Transporter).

The application due date is April 15th, 2019. The information flyer for the program is attached. Also attached is the WorkTrain application (to be used to apply for the Health Train program).

Please encourage candidates to stop by the SUNY Syracuse EOC Office Admissions Office at 100 New Street, Room 200, with their IDs and income information (2017 1040, PA statement/letter, SSI letter, or release papers, etc.) to fill out the application for the program.
