The role of the Disability Program Navigator (DPN) at CNY Works is to facilitate seamless and comprehensive services to persons with disabilities.
Services:
Referrals to local support services that help meet your employment goals such as:
- Social Security Work Incentives
- Benefits Planning Assistance
- Medicaid Buy-In Program
- Housing Assistance, Transportation Options
- Work-Site Accommodations
Development of linkages and collaboration with employers to facilitate job placements
